Macular hole surgery without the use of posturing postoperatively can be arranged for smaller holes. Many patients find the most traumatic part of the operation is the posturing regime used in the first week however a recent multicentre randomised controled trial from France suggests that a shortened regime of posturing provides a better outcome than no posturing especially for larger holes. For this reason a regime of 6 hours posturing a day for 7 days is recommended.
